Collaboration agreement

The FixOurFood in Schools network is part of a project aiming to transform the Yorkshire food system for dietary and planetary health. This study is being carried out by the University of York and is funded by the UK Research Council ‘Transforming UK Food systems’ programme.

This collaboration agreement indicates your school is agreeing to take part in the FixOurFood in Schools network and the purpose of this is to record your agreement to be involved, as follows:

1. Your contribution to the FixOurFood in Schools network will be conducted to the highest professional and ethical standards, and will comply with all laws, regulations, approvals or codes of practice that may apply.

2. You will comply with all reasonable requests of "University of York” in relation to the study, and will give the study researchers such access, and assistance as it may reasonably require for the purpose of the FixOurFood in Schools Study

3. Throughout the study you should continue with all operations as usual within the school (i.e. we are not asking you to change anything you would normally do except that we may influence how children collect and return their lunch meals if you are part of the school researcher visits).

4. You will ensure that school teachers and staff have sufficient opportunity to participate in the study (if willing) and, if part of the school researcher visits, the member of staff chosen to take part in the interview has sufficient time available during their working hours.

5. In the course of the FixOurFood in Schools study, a range of data from the schools will be collected. This data will be gathered and stored in accordance with the FixOurFood in Schools protocol and requirements of the Research Ethics Committee (REC) required by the research funding body. You agree that collected data will belong to the University.

6. You are able to generate any relevant publicity regarding your participation in the FixOurFood in Schools network, but we would ask you to share this with the research team. We would also ask you to acknowledge the FixOurFood project.

7. You may withdraw from the FixOurFood in Schools network, with agreement of all involved.

By continuing with this form you are confirming your agreement to the terms of this collaboration agreement, for authorisation of the study in your school.
